Biometric Data Notice and Consent Form

Instruction: Include this provision in any form or log-in sheet that an RPM guest or visitor signs before they are subject to screening. 

 Notice and Consent of Personal Data: We use facial recognition and temperature screening technologies to control entry and exit to this facility and for related health, safety and security purposes. Automated temperature screening is a condition of entry for all employees, temporary workers, and visitors.  By signing this document, you understand and agree that this facility may use cameras, kiosks, and similar technology to collect, retain, and use information derived from the scanning of your facial geometry (i.e., biometric identifiers) for these purposes.
